I think the imposter is going to assasinate the guy in the middle with whatever the other doctor is slipping into his pocket.

That, or it's a movie about the first pregnant man in history, as you can tell by the too-tight suit.

I didn't remember these little things were in this film till I went through looking for stills. I think it's fair to say they aren't a major part of the film
