well, I tried, filled out the answers, and for the gps coordinates, I found this which lists Columbus as

Apparently I got 8 right and got a new set of codes in an email.

I don't remember and don't feel like looking up where I got all the answers but if you want to a specific one just ask.

New codes are attached, haven't looked at them yet. (Literally haven't unzipped the file)

ETA: Not sure what to make of the audio files, I thought they were new codes but they seem to be story related more than anything.

"Surveillance" is listed only in the Columbus-Only link - it is not available in the Internet-Only link. Could that be what happened?

So did I, I just put the SM answers in the surveillance box.

And what emato said is correct in that "System Maintenance" is called surveillance on the other screen. However, both forms are the same, which basically means they only had one form for both pages. Tad sloppy, but forgivable.
