There is so much access to water everywhere TO has shown up, I would not be surprised if he could come to this dimension or whatever through water and/or people have to go through it somehow to get to his.
Even when Jay was staying in hotel rooms, the hotel had a pool, didn't it? I don't know for sure, but this is very intriguing to me.

And what about that room that Alex and Seth investigated in Entry #22 and The Operator transported Jay to in Entry #23?
That was the pump room for a pool, which has another affiliation with water.
It's all coming together!

Oh god, and think about where Alex killed that guy. Those tunnels tend to be used for allowing excess water run off

In the Season 1 DVD commentary Troy said that they filmed entry 22 in the basement of an abandoned mental institution, most likely the "abandoned hospital" from recent entries. I really don't know where people got this pool house idea from. The basement of doom doesn't even look like the basement of a pool house. Most pools just need a heater, a pump, and a filter, they don't need giant ass boilers. And most pool houses aren't that big. I live down the street from a public pool and their pool house could probably fit inside my garage. Also, if you watch Entry 56 the banisters of the staircase going to the basement look just like the banisters to the stairs in entry 22/23.

Trying to wrap my bead around Entry #13. Isn't this the first time we see the x-o operator symbol? Also: sound of running water at :57 right before sound distortion and TO appearance.

We see the XO in Entry 8, among Alex's drawings.

The abandoned place is 13 is a water association, of course. I think it's part of what misled people about the Basement of Doom, as they assumed it was inside one of the buildings there, when in fact the guys were illegally trespassing in the Basement.

If the boiler room is in the hospital, then that might be what's behind the door. We now know that water is how TO travels from his world to ours. He probably used the boiler room to follow Tim to the hospital when he was a kid, which is why Tim was screaming that he was there. That's also likely what Tim wants to show Jay.

You know, just the one Tim was screaming at and deathly afraid of in the entry this thread is about. I can see how that confused you, what with all the doors in that entry.

ETA: Also, the boiler room. The boiler room is what might be behind the door.
