You were recently given a trial sample of the drug Fluoxetine(Prozac) by your physician. Our company supplied your physician with these samples. By receiving and using the drug your name was added to our database for further followup and research. We are a new company in this market and appreciate your time and confidence.

The following is a short questionnaire that will help in the development of new and improved drugs. *Please answer the questions honestly and with as much detail as possible.

1. Were you diagnosed with depression by you physician?
No. He diagnosed me with OCD complicated by post-traumatic stress syndrome.
2. Did you experience any disturbance in your sleep patterns?
Please describe.
I was hoping the constant sleepiness would abate. So far, it seems to have diminished slighty, but not enough.

3. Did you experience nightmares or night terrors?
Please describe.
I am assaulted by constant flashback dreams to my childhood home. An enormous discrpency in the dreams from my memories is that my parents have been replaced and I am both my father and mother.

4. Did you experience any violent behaviors or thoughts of violence?
Please describe.

None.

5. Did you experience any feelings of grandeur or feel that God was communicating with you?

No more than before.

6. What dosage did your physician prescribe? Did you exceed the prescribed dosage?

90 mg sustained release
7. Would you be interested in participating in a further study of this drug?
If so, you will be very well compensated for you time and all
medications as well as visits to your physician will be covered.

No.
I look forward to receiving your completed questionnaire and hope that you will become a participant to further study this drug.
