So I cannot seem to find anything of note in the patient file. I can make out the name, but that is about it. Tried messing around in various programs but cannot seem to pull anything else out of it.

On the form, it looks like
Spoiler (Rollover to View):
"Oakford Ave," zip code 21216
for part of the address and
Spoiler (Rollover to View):
"Baltimore City College"
is under education.
Making out that email though... I've got nothing.

Yeah I really want to check the mail as well but no luck on getting a clean image of it.
For the QR code I just darkened it with photoshop and then used neo reader on it. Breaking the vig encoding was easy due to the word may and case being there

That may be the key to this one, you've got further under the ink than I did. I'll try some more mañana but we'll prolly be on to the next by then.
Mr. B:
NeoReader... "The most simple, fast and highest performance QR Reader on the market – and FREE!" is obviously better than the two I had in the arsenal.
Muchas gracias for the tip!
And yes, I dined at Los Amigos with tres, maybe cuatro of their finest Margaritas. So sue me más tarde.

Insults to my skateboarding ability aside, Kyle replied to the post I gave ("Snoopy"), pretty quickly.
I'm curious though; if he's the CEO, who's his boss? And if there's other patients, would their numbers follow a logical order (CASE ID 002 etc).
And we might be ousted soon IG... "By the way, I haven't seen you.. or almost any of you at the company meetings. Or around the office. At all. It's like you're not even here. Huh."
Think it's time we start saying "kool" and donning shades, guys

Kyle was very salty last night! Thanks for the backup, V!
So Sarah uploaded another rock picture and

Going to that link shows that the folder is set to private. It instructs you to log in to request access to the folder, so I created an account and requested access.

That email is tough. Some guesses:
lionelSPLAT S__p.com
LionelSPLAT Exip.com
EDIT to add: It would seem Eoip.com is a real thing with skateboarding and a paper cup of coffee. Sent Lionel a email asking if he's ever seen Breaking Bad.
No bounce back yet.....
Edit 2: Whois says its been around for awhile, so probably no go on Eoip.com

So it looks like the file is available now. A movie with some hidden messages. Going to download a program to dissect it now.

Interesting video at
Spoiler (Rollover to View):
http://www.mediafire.com/watch/h1y0h3q14jgutd8/insertion_1.mov
Few things flash on it, but what caught my interest most were three things:
1. Massive list of companies, people etc.
2. EGAREVA
IICSA
YRANIB
The words are backwards, so they're:
AVERAGE
ASCII
BINARY
3. Also this flashed up: "email to bluemindincSPLAT gmail.com
subject 1100…"
Lastly, the folder Sarah has it saved under is "jCORP" despite the user being Bluemind Incorporated.
Thoughts? Specifically, what the words could refer to?

Not sure what they refer to, maybe something that will be emailed if you email that address? Can I ask what program you used to view it?

I just played it from the mediafire link. Took a few minutes to pause it properly but just did that a few times until I could read it

Cool, thanks! I have an idea, not sure if it makes any sense, but since it said subject 1100... that looks like binary. Like 1100xxx and we are supposed to figure out the rest of it?

I downloaded the video and here's the full list.
I noticed the time on the corner. I don't know what it means or why it is there, but I pasted the exact time when the list starts and when it ends, just in case.
