Why would he just kill Jessica if he has been helping her get passed the virus for so long? That doesn't make sense to me.

Some say that he was taking Alex's suggestion, and is going to kill whoever is left. Also, apparently, there the sound of ambulances right after Tim was in a coughing fit and was driving in the car.

I've tried my best to hear the sirens that people have been claiming are in the background, but I just can't hear it. I hear some little noise, but it doesn't sound like sirens to me

They're definitely there.

I'm not convinced the sirens are supposed to be part of the story. If they were, then maybe this idea has some truth to it. Perhaps when Tim goes into his coughing fit he does. Also, when he starts coughing in the car, we see him bring his pills up, but doesn't take one; perhaps he's also done trying to fend off the Operator?

The sirens just sound like wind to me.
Tim wouldn't kill Jessica, mostly because I don't believe he thinks Alex solution would work.

I'm no sure why people are so sure he didn't do anything to Jessica.
Sure, Tim wouldn't do that if he'd protecting her all this time. But would a Tim that goes into a coughing fit with crazy static on the camera because he's falling under the Operator's influence do it? Possibly. Also consider Jessica staring at a fixed object behind Tim in the final frames we see of her. To me, the coughing fit is there to imply that the Operator was there, not just to tell us "oh, Tim's still stick...bless him" not to mention the abrupt cut.
The static noises during that coughing fit are classic TO ambience too. There's static when people have coughed before, but not _that_ kind of static.
Alex, TO's supposed "host" died in #86. Maybe it latched onto Tim again? We've seen Tim fight TO off before, but was he able to again? Who knows. Also even if Tim didn't kill her what if TO did something to her?
He might not have killed her, but I don't agree that it's obvious he didn't either. The coughing + static too strongly implies TO activity to pretend that nothing crazy could have happened after the cut.

There's a large amount of evidence pointing towards him doing so. Why else would Tim meet with Jessica, coincidentally after Alex tells him that the only way to end T.O. was to eliminate all of its victims, while KNOWING he's still infected w/ the coughing? During the scene itself, the high amount of static + coughing, a sign of Tim going Masky/Proxy-Mode, can clearly be seen. When in Masky-mode, Tim has a tendency to be violent- as seen in the entry where Jay goes looking for Tim and gets violently attacked as a result. After the otherwise unexplained static/jumpcut, Tim seems to have an expression VERY similar to Alex. Blank, cold, straight-faced-- almost exactly how Alex was after his killings. In addition, most viewers claim to hear sirens in the background at ~4:08.

Although there is evidence pointing toward it I don't think it happened. I think the whole Tim ending was to contrast his viewpoint vs Jay and Alex.
Alex: Kill everyone to rid the world of the operator.
Jay: Find Jessica and possible reintroduce her to the operator.
Tim: Keep Jessica hidden, stop Alex, and then leave presumably taking the operator with him.
I'm not wild about the ending, but that's the only way it makes sense to me. I can't see him suddenly buying into Alex's plan at the last minute.
You could make the argue that the operator is controlling Tim and makes him kill Jessica, but he seems the most resistant to the operator so I can't buy that either.

I'm going with "Yes" because of the conspicuous lack of explanation for what happened in that scene.
Also, I don't buy Tim's claim of being resistant to The Operator. Despite him popping those pills, he doesn't get rid of it in Entry 86 & ultimately shanks Alex to death right after it shows up.

Tim didn't seem to have any sort of weapon going into that meeting, was wearing the same clothes driving away from it as he was waiting for it, and showed no sign of being in any sort of struggle.
Maybe he was considering it when he went to meet her, maybe he did genuinely just want to say goodbye before he vanished (if only to stop her trying to work out what happened to him, which could cause her to get dragged back into all this), but either way I don't see him turning around and killing her with his bare hands (or even a knife) after a coughing fit that serious - he'd be too out of breath and exhausted.
But that's just my gut feeling, for the most part the conclusion seems constructed to allow you to support your own take on the ending if you really want to.

It doesn't really make sense that Tim killed her right after the video cuts out because I mean, how can you just kill someone out in the open in broad daylight? If he did that at the least you'd think when he was driving away from it he'd be going really fast, but he seemed at leisure in the final part. Also he would've had to just strangle her without getting blood/bruises/whatever on him seeing as he looks pretty clean in the next scene.
How do you guys that are voting yes even imagine that he did it?

I really don't think he has.
He has been protecting and helping her, even keeping her secret from Jay.
I think we have been shown to oposite sides of the same: Alex, the one who thinks that everything has to end with death (and who dies), and Tim, who eventhough he knows it is going to suck, has the will to live and learn to face and carry on with all that has happened (and that includes not killing Jessica).
