Nice, another chemistry puzzle (playing to my stren...well, my career anyways). I imagine that this may be a code similar to one of my favorite chemistry lecture games, using the abbreviations of the elements to spell out funny phrases (ie. mostly rude words). Like 9, 92, 6, 19....or the annoying fashion brand 9, 29, 19. hard to tell without the card/a transcript.

No transcript i'm afraid, there appears to be nothing written on the card
Spoiler (Rollover to View):
However the answer page states, Please give two word answers. Then gives you four answer boxes

Sorry, I meant a transcript of the numbers, which seem to be too hard to discern at the lower resolution/fuzziness of the image.

Hahaha, nice transcription there romper2004, you just have the 3 and 8 early on on the wrong lines. You should be able to answer this one now using my method!

I get the following 'translation'...
Spoiler (Rollover to View):
one precise replica two kind of eucalyptus three south west hi tech america four facial ition of alcoholism
Either I've done the final bit wrong, or there maybe a spelling mistake on four.
So I tried the following answers in two attempts:
Spoiler (Rollover to View):
1. Carbon Copy
2. Gum Tree
3. Las Vegas
4. rehab centre/red face
I think 1 and 3 are correct, not in any way sure about 2 and 4

Spoiler (Rollover to View):
i'd have said three was silicon valley.
which, given carbon copy, leads me to suspect the answers all have an 'element' in them

Good shout. How about...
Spoiler (Rollover to View):
Silver Mallet or Red Ironbark for the type of eucalyptus - I would imagine that it is more likely to be Silver Mallet
still not sure about 4

The code was fun, but I've been stuck on #4 for several hours. And to go along with your #2's, how about these:
Spoiler (Rollover to View):
Silver Dollar or Silver Gum
This one's killing me!

I'm guessing they would have a few variations of correct answers for 2....but 4, yes, is a bit of a killer.

I'm glad it's not just me stuck on 4! I've been through the periodic table trying to use that to help me find a two word answer - but no joy!

I feel embarrassed that I can't identify or remember the medical term for 4. My medical text books are no more useful than google for this one. Perhaps our Oxford correspondent will have a better memory.
Spoiler (Rollover to View):
Since copper and iron metabolism is awry in alcoholism, I think they are the most likely elements to be in a term which is a facial indication of alcoholism. It could be that they are thinking of fetal alcohol syndrome, but of the predominantly facial features, I can't think of one with an element. I better go back to the periodic table myself and work through.
Spoiler (Rollover to View):
EDIT: Decided to concentrate on the nose and found "copper nose" in a list of archaic medical terms.

The 'silver dollar' and 'copper nose' combination doesn't work.
