Could the first part of the cipher possibly read "The good doctor"...?

Did a quick look up, and even though that phone number isn't working, it states that its in the general area of Hudson, Ohio. Is that where the hospital was?

Z = A
Y = B
N = I

Sorry, I'm a bit on the fritz at the moment and currently at work, so I'm running back and forth... but if you swap out these letters, some interesting words come up (such as "good", "doctor", "habit", ~~"go"~~ , and "dead")

Aw, dang, I was so close to solving it myself, too.

Although I'm not sure what this means now. At face value maybe it means Corenthal's in the same situation as Jessa? And what's the true nature of Evan's / HABIT's mental issues? I have a feeling the latter question might be answered when the blacked-out word before Evan's name in the letter from Corenthal is revealed.
