Warhorse: I think the allcaps name makes sense if his whole name is capitalized, it makes it stand out and makes each letter clear (so no I/l problems).

According to the wife, who looks at these things for a living, it doesn't typically occur in medical records. So, at the least, it's an irregularity that can be somewhat justified.

Direct quote from the wife:
" also they almost NEVER address them by their first names in the records
(11:09:15 AM): it's always Mr. blah blah or the patient
(11:09:27 AM): again this could be just them not understanding medical records"

This seems more like a personal letter about the patient rather than a real medical record, so I guess it makes a bit more sense to call the patient by his first name - also, the doctor seems kind of troubled and not completely focused (all the typos/mistakes), so I guess some of the formalities could have just slipped?

Also we would not have been able to realize that the patient from Fairmount = Evan if the letter didn't use his first name, so it could just be that.

Two thoughts.

One: Someone should take all the typos/mistakes out of the document and see if those words reveal anything (Not me. I'm at work).

Two: The only way that this document is referring to the Evan we see in EMH is if the slender one took him on a stroll forward in time.

Oh, I didn't mean that the Fairmount Evan is literally the Evan we know, just that we wouldn't have been able to make the connection between the names if the first name wasn't used in the letter.

The mistakes I see in the letter don't seem to mean anything at first glance (I'm not looking incredibly carefully, though, I'm doing homework):

Has is capitalized in the middle of a sentence
"arrise"
"know" where it should say "known"
"dead" where it should (presumably) say "dear"
"and" where it should say "an"
"concious" where it should say "conscious"
"realse" where it should say "release"

I don't think there's any kind of hidden meaning here and it's just supposed to go along with the doctor's paranoia/sleep deprivation.

Vinnie Everyman!!!! :O Oh em geeeee... but EVAN being the last name (or part of the last name) does kinda make sense, since theres a blacked-out part before EVAN and not after.

I assumed it was supposed to be "[LAST NAME], EVAN" and the last name was what was blocked out.

That's how it's typically represented in medical files.
