I've always been under the impression that the EMH crew has been making everything up as they go along.

Actually I think they have some sort of timeline in their mind since there is a lot of bits where you'll see foreshadowing.
Or our minds/words are just putting the foreshadowing there and the boys work off it and you're completely right XD;
