Definitely 19, with 18 as a close second. I had trouble sleeping for two days.
I agree with warbear for 19, a person in your room is mich more likely than SM.
And also seeing masky just sitting and staring was much more crrepy than the subsequent attack

Creepy: Entry 6. HE WATCHES YOU THROUGH YOUR WINDOWS AT NIGHT
Terrifying: Entry 26. Maybe It's my fear of things getting in my house, but MY GOD. The one time I actually jumped in this series was when The Operator appeared in Alex's house. Brrr.

WOuld have to say 1, 6, 29 or Fragments.

Theres something about the pose in 29 that sends shivers up my spine, it's so inhuman and menecing. But nothing can truly recapture the terror from the first few videos.

This said, Fragments was just raw terror the first time I viewed it.

Entry #23. The first "OHSHI-" moment was when I realized Jay was apparently literally entering and leaving the same room at the same time; the second was when I realized he was losing a LOT of time in between these "jumps". Third was "Oh crap, it's dark now, all bets are off." Then there was the Slendybasement, and the "echoes". It was just a total mindscrew from beginning to end. I hate it when natural laws just decide to up and take a vacation.

Second was Entry #19, Masky all suddenly pulling a Batman out of nowhere standing on Jay's dresser. Third? #14, the visual tear, and then that arm just slooooooowly pushing open the door....

Entry #18 100 and ten percent.
I had never seen MH before and was watching them from start to finish at about 11 at night with all the lights on.
I was on my bed leaning up against the wall. My bed is right in front of my door, which was open, and there is a very long hallway connecting my door to the bathroom door on the other end.
The bathroom door was also open. If I looked up from my laptop screen it was basically the same perspective that J has looking down the hallway as Masky lunges towards him.

I've never shitbrix so hard before in my life, I couldn't even watch episode 19 (the most recent update at the time) I was so scared.

Entry 6 is probably the second most scary entry. I know what to expect when I watch it, but it always freaks me out.

Entry 23 is the scariest. Damn atmosphere.

Aside from the noise distortion, #18 and #19 didn't scare me at all. After seeing the Slender Man and pure freaking embodiment of terror, the Masked Man seemed like a huge joke to me.

Most scared that I can remember being was watching Entry #14. That freaking entry. I was frozen in fear. Eyes stuck on the screen... hand glued to the mouse... That was the peak of the series for me.

Then would be #22 and #23(minus the not-scary Mask). The mind-screws plus the BASEMENT OF DOOM. Very similar and very scary.

For totheark, the worst for me was definitely Admission. The visuals were horribly creepy, the sound was horribly creepy. Then the Operator walks by all unnatural. How'd totheark film him like that?! Then that door. That abominable, frakking door! And then it slammed shut! Then opened on its own again!

I felt so terrified, like a shrunken little insignificant chibi of myself sitting at my computer chair, waiting for him to come kill me. I defecated a masonry.

This one's weird and I'm probably the only one who'll mention it, but Program. That twisted soundtrack (so creepy) with the clicking noise of an old-fashioned film projector with the image of the scary building with the flashing zeros and hyphens... It effectively struck that primal 'wrong' chord of mine that we all know about.

I'll probably sound like a weenie, but Return.... I can't even watch the whole thing. I've gotten used to almost every other entry I found creepy except that one. And Addition. The sound distortion just makes me queasy.

Definitely totheark's Signal video.

The visuals could have been anything at all, it's just the voice that does it. A hushed whisper with a slightly sinister tone saying that stuff gives me chills every time

Entry 22, The raw suspense and then the hyper audio distortion and creepy camera cut really scared the hell out of me, the shadow effect with slendy near the end almost gave me a heart attack.

Entry 26 definitely, the idea that they were just having a freindly conversation when BAM slendy in your home! Also the "help me!" At the end really toped off the fear factor.

Return was by far the scariest totheark video. The way slendy just lumbers towards the camera with the lights flickering REALLY creeped me out.

Tothearks actual appearances arn't terribly scary compared to slendys, but I got pretty freaked out in entry 15 when totheark was jumping all around the bed. The idea of a masked stalker in your room while you sleep really freaks me out.

That was #19. I don't remember Tim jumping around a bed in #15.

Anyway, for me it HAS to be 19. Just because when I first started watching it, it scared the hell out of me and I literally closed the page when he got really close to the camera.

Otherwise, Return is a mixture of "woah wtf" and "Wait what?" for me. I'm always trying to figure out what exactly is going on.

And don't get me started on Admission. Bricks were everywhere and sleep was not had for days.

Definitely have to go with Addition as most unnerving and creepy. I have an innate fear of being watched/stalked. And Entry 6. Just the way he walks in front of the window so slowly.../shiver.

Terrifying had to be entry 26. I was not expecting Slendy to just be chilling in their house.

I can't believe no one has said Entry #####. The music, the voice, the visuals, the everything oh my God. A few had startled me, or unsettled me, this is the first and still only one that genuinely SCARED me. And it still does.

EDIT: Now that I think about it, totheark Warning also creeped me out. I have an easy button to push to scare me, and it's things moving unnaturally. (Think the blonde chick in Gothika, or pretty much anything in that Legion movie that I refused to see because of the trailer.) So that mannequin moving around the way it does in Warning gave me chills.
