Ive seen that same cat held by a man. Look closely at the wrists. I think a small child originaly held the cat.

Perhaps, although a small woman holding the cat closer to the camera could possibly have produced something like this.
My cousin has a Maine Coone that weighs in at 18 lbs, with very little fat. The cat's bone structure is massive . I think her ancestors must have interbred with lynx. My cousin is 5'1", so when she holds her, it doesn't quite look as ridiculous as this, but it comes close.

Me too, it's been around for a couple years.
And yes, it's a Photoshop. They had a short article in it in Cat Fancy a while ago (my sister subscribes and thought I'd be interested, being the geek that I am).

Just a talented Photoshopper. It's not that hard to do, it would just take a while with the airbrush.
1) Take photo of cat, really close up.
2) Take photo of human holding nothing.
3) Cut the cat out of its photo.
4) Paste the cat onto the human's picture.
5) Spend too-long with the airbrush (and other various tools) until it looks good.
6) Spread it around the internet.
