I'm doing frame grabs tonight of both forms. From what I was looking at last night, it appeared that one part was for medications he's already taking, and then there was an allergy line written at the end. It doesn't even mention what drug causes the hives. It just has the allergy box checked. For the most part, it's a fairly standard admission form.


We already got frame grabs, and a filled out form back on page 19...

amoxicillan and Lodeine cause Tim to break out in hives. He is presently not taking any medication, hence the N/A in the medication box.



Those are not screenshots. That is the picture or pdf of the form the guy made and filled out. It's also his best guess at what he can see. I'm going to get actual screen grabs of the forms. If I want to waste my time with it, I will; because what I saw last night (and stupidly forgot to save), he's got the form wrong in a couple of parts.

The origins, perhaps? Seems probably that "reference" point directly to this forms and we can "look at the origins" in this.

The more I think about it more I am convinced, that TO is linked to Tim from childhood (if not created it himself), it is even possible, since before his admission to the hospital. His parents (his mother) they leave in the institution and they turn away of him, perhaps out of fear. Who knows if their home did not result burnt down and even if his father died as a result. I imagine a broken home, an alcoholic father, a wife resigned, an abused child and malnourished, fed by hate and revenge fantasies channeling through his friend "slendyrag". In the hospital things subside until someone or something does not like and the nightmare begins again.

 		OMG. So this will probably not be correct either way, but I believe that there is a chance that Slendy used to be an "Operator" for the hospita but did something terrible (like starting a fire) and got hanged like how in that ghost story Alex once said. Maybe that explains Jay's apartment fire or Alex did it as a task. Tim may have gone to the old building when it still was in use. Hmm, Slendy does like children so maybe he was a paedo in his lifetime? Or maybe he had his face burned off in a fire and the doll is what he hopes to look like again...

Stupid theories, but interesting nevertheless.

It would be quite EPIC if the twist revealed that Tim is actually the one who was stalked as a kid, the one who brought TO to the lifes of Alex and Jay. Mindblow.

Ok, I took multiple frame grabs of each part of the form. My memory appears to have failed me. I compared my frame grabs with the form the other guy made. I got the allergy field and medications field mixed up because the form is just so bizarrely specific in what it asks for, unlike even admission forms from 1995 that I remember my mom filling out for me when we would go to the doctor.

So, yeah, he was pretty accurate with the form recreation. I was just apparently derp. Razz But, I did accomplish what I wanted to do, which was provide actual screenshots to compare and contrast. Very Happy. Here are the best ones from each part of the form.

Hasn't it kind of been suggested that Tim was the earliest known afflicted with slendersickness symptoms?

I don't actually know anything about House of Leaves, but that is the other thing, I already know from season 1 that seemingly mundane pieces of the setting can be more supernatural than I strictly care for.

 		So I watched the video again and realized I was wrong, that the Operator didn't show up until after Jay was making his way back through the tunnel. I'm guessing that "His lies in the dark" meant what was listed in those forms, but what is Tim lying about? Is he not better, or is it like others have suggested and he's the first one to introduce the Operator into everyone's lives? If that's the case, how did OP latch onto Alex, and why? I still don't know for sure if Jay teleported or if somehow he managed to get away, but what I'm hoping will happen is either Tim will come to his senses and show up to help Jay or TTA/Hoody will finally come out of the shadows and explain himself.

Well, we have seen him coughing and taking pills in Brian's house in S1, if that's what you mean. But from that point to this, the suggestion of Tim being Slendy-stalked almost his entire life, it's way way different.

 		A thought occurred to me while catching up on this thread. Maybe Jay had more or less put any thought of TO out of his mind, and the doll reminded him enough for TO to appear (I remember theories that Slenderman appears when he's thought of). If this is the case, I don't think Jay should be reusing his camera anytime soon.
