Don't know if this will help, but these are the only notes referenced in the midi file:

Is it possible it is using some sort of music notation? E is the first note and the song title is called //CLASSIFIED//E=mc3//

So if:
..\images\mc3-???-???-???-???-???-???-???\classified.mp4

Then:
..\images\mc3-???-???-???-???-???-???-mc3\classified.mp4

The uploaded 'wav' file contained a gif file inside of it, though every other byte of information was 0 (or null). Removing all the nulls and saving the file as a .gif results in the above image.

OK, what seemed to me like a gamejacker at first has now led us to this:
